SAS Care will provide a service which is person centered, personalised, enabling people to make informed choices. Aim
The aim of SAS Care is to provide individualised support to people to enable them to fulfil their potential as individuals and citizens of their community.
This service will be provided by a team of professionals who have been instructed in the methods of implementing interventions, which encompasses the philosophy of the service. Recognition of the importance of being in control whilst receiving support interventions will be the focus of our instruction as well as accepting the importance of good communication skills, interaction in the community and respecting the notion of choice.
Objectives
Person centered assessment of needs by an experience assessor using an approved tool from point of referral to care delivery will enable our people to make informed choices.
- To provide support workers who are experienced in meeting the assessed needs.
- To provide a service which is person centered and person led.
- To provide clearly stated objectives about what is required on a day-to-day basis.
- Needs and interventions are made in a multidisciplinary context, deriving resources and information from the multidisciplinary team.
- To monitor, record and evaluate interventions and plans.
- To be flexible, and adaptable to respond to changing needs of people.
- To provide continuity of care by assigning a personal assistant to coordinate interventions.
- To continuously evaluate the operations of the organisation.
- To provide support workers with regular training this meets the requirement of the Care Quality Commission.
